These Terms of Service ("Terms") govern your access to and use of the website creativehealthquotes.com and any services provided by Creative Health Solutions LLC ("Creative Health Solutions," "we," "us," or "our"). By accessing or using our Site, submitting a form, or participating in our SMS program, you agree to be bound by these Terms. If you do not agree to these Terms, do not use our Site or services.
1. Age Restriction
You must be at least 18 years of age to use this Site, request a quote, or opt in to our SMS program. By using our Site or services, you represent and warrant that you are 18 years of age or older. We do not knowingly collect information from anyone under 18, and our SMS program is not directed at minors.
2. Services
Creative Health Solutions is a licensed health insurance brokerage. We help individuals, families, self-employed professionals, and small businesses compare and enroll in health insurance plans across all 50 states. We do not underwrite or issue insurance policies; coverage is provided by licensed carriers.
3. Quotes and Recommendations
Quotes provided through our Site are estimates only and are not binding. Final pricing, eligibility, and plan availability are determined by carriers based on underwriting criteria, household composition, income, and location. Subsidy eligibility is subject to verification by the federal or applicable state marketplace.

4.0 Required SMS Disclosures (CTIA)
Creative Health Solutions
Creative Health Solutions sends SMS messages to individuals who have requested a health insurance quote, scheduled an appointment, or opted in through our website, by phone, or by text. Messages include appointment reminders, quote follow-ups, plan recommendations, enrollment updates, account notifications, open-enrollment announcements, carrier offers, and educational updates about health insurance.
You can cancel the SMS service at any time. Just text "STOP" to the (321) 872-4940. After you send the SMS message "STOP" to us, we will send you an SMS message to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from us. If you want to join again, just sign up as you did the first time and we will start sending SMS messages to you again.

Carriers are not liable for delayed or undelivered messages.
As always, message and data rates may apply for any messages sent to you from us and to us from you. You will receive approximately 2-6 messages per month, with volume varying by your stage in the enrollment process. 4.1 Program Description
Creative Health Solutions operates an SMS messaging program for individuals who have submitted a form on our Site, contacted us by phone or text, or otherwise opted in. The program is used to send transactional messages (appointment reminders, quote follow-ups, enrollment status updates, account notifications, and customer-care responses) and marketing/promotional messages (plan recommendations, open-enrollment announcements, special carrier offers, and educational updates about health insurance).
By opting in, you understand that the SMS program is used for both transactional and promotional purposes related to your health insurance inquiry and enrollment.
4.2 Consent
To opt in, you must affirmatively check the SMS consent box on our website lead form, text our business number directly, or provide verbal consent during a phone call with one of our licensed agents. Consent to receive SMS messages is not a condition of purchase of any product or service. You can purchase health insurance through us without consenting to text messages.
4.3 Message Frequency
Message frequency varies depending on your stage in the enrollment process and your communication preferences. Subscribers typically receive between 2 and 6 messages per month, although volume may be higher during open enrollment periods or while actively reviewing plan options.
4.4 Message and Data Rates
Message and data rates may apply. Charges for SMS messages are determined by your wireless carrier and your individual mobile plan. Creative Health Solutions is not responsible for any charges your carrier may impose. Please consult your wireless carrier for details on your messaging plan.
4.5 How to Opt Out (Text STOP)
You can opt out of our SMS program at any time by replying STOP to any text message you receive from us. After replying STOP, you will receive one final confirmation message and no further marketing or transactional SMS messages will be sent to that number. You may also opt out by replying CANCEL, UNSUBSCRIBE, QUIT, or END.

4.7 Carrier Liability Disclaimer
Carriers (including but not limited to AT&T, Verizon, T-Mobile, Sprint, U.S. Cellular, Boost, Cricket, MetroPCS, and Virgin Mobile) are not liable for delayed or undelivered messages. Creative Health Solutions and its mobile messaging providers shall not be liable for any failure of any text message to be delivered or received in a timely manner. Delivery of SMS messages is subject to effective transmission by your wireless carrier and is not guaranteed.
4.8 Privacy and Information Sharing
Your phone number and SMS opt-in consent are governed by our Privacy Policy. We do not share, sell, rent, or otherwise transfer your phone number or SMS opt-in consent to any third parties or affiliates for marketing or promotional purposes. Phone numbers collected for SMS consent are used solely for direct communication between Creative Health Solutions and the subscriber. 4.9 Supported Carriers
The SMS program is available on most major U.S. wireless carriers. Carriers are not liable for delayed or undelivered messages. T-Mobile is not liable for delayed or undelivered messages.
